INTRODUCTION:WHAT IS MAINTENANCE? AND WHY IS IT IMORTANT?Maintenance in simple words is a set of activities which help keep plant, equipment, machinery, vehicles and other facilities in good condition.Every manufacturing organisation needs maintenance because, machines breakdowns, parts wear out and buildings deteriorate over a period of time of use.Maintenance is an important factor in quality assurance . Inconsistencies in equipments lead to variability in product characteristics and result in defective parts that fail to meet the established specifications.Good maintenance management is important for the company's cost control.Organizations like airlines and oil refineries have huge investments in the equipment. Equipment failure will be disastrous for such companies.Let us take a look at 5 such incidents caused due to poor maintenance, and the disastrous impact it can cause not just to the companies involved but also to the life of many people.THE BHOPAL GAS TRAGEDY:
Night of 2-3rd December, 1984.
Bhopal, Madhya Pradesh, India.
The tragedy took place due to the uncontrolled leakage of a poisonous gas Methyl Isocyanate known as MIC into the atmosphere from a storage tank in a pesticide manufacturing plant.
5,000 dead; 20,000 seriously injured and 2,00,000 seriously affected by MIC.
Poor maintenance of plant and ignored security measures, due to which the leakage was not detected and the chemical continued to leak into atmosphere.
Bhopal Gas Tragedy is one of the world's worst and fatal industrial accidents which took place due to the uncontrolled leakage of MIC. The plant was owned and run by Union Carbide, Inc. and its Indian subsidiary, Union Carbide India, Ltd.
MIC (Methyl Isocyanate) is a lethal substance that is highly volatile, reactive and toxic. Now that the chemical is volatile, its fumes start spreading across the area along with the prevailing winds.
It was concluded after the investigation that the traces of water might have entered the MIC storage tank and caused the chemical reaction which caused the leakage of this highly toxic chemical. Due to the poor maintenance of plant and ignored security measures, the leakage was not detected and the chemical continue to leak into atmosphere.
