Emergence of Professional Based Care
In today’s world of rapidly advancing medical technology, the health care consumer is demanding high quality care. Additionally, the face of the health care consumers is also changing at a rapid pace. At the same time regulatory and governmental agencies are pushing health care providers to not only provide this high quality care in a cost effective manner, but to do so in a way that ensure equality to all clients. To meet these challenges, it is imperative for health care professionals to provide this level of care in a way that is effective, efficient, and patient-centered. Currently, medicine is dominated by males at 67.6 percent and females occupy 32.4 percent of the profession (Bankston & Glazer, 2013). These numbers contribute to the notion that physician are superior to nursing, when in fact each discipline brings a valuable skill set to the table. Nursing has gone to great lengths to develop a unique foundation of nursing ethics, theory and knowledge, make their practice distinctive from medicine. These efforts ameliorated nursing as an individual profession (Tymkow, 2013). It is quite possible APNs fear that through collaborative practice this individuality may be blurred.
The Council on Graduate Medical Education (COGME) and the National Advisory Council on Nursing Education and Practice (NACNEP) recognize the value of patient-centered collaborative practice, and also acknowledge the existing gaps. These two agencies recommend interdisciplinary education to break down barriers. This type of education brings the two disciplines together with the objective of socialize the APN and physician into working as a team, where they speak a common lauguage, focus on core fundamentals such as quality of care, patient safety, ethics and leadership (Bankston & Glazer, 2013). Another method the COGME and NACNEP implemented to encourage professional health care providers to work in a collaborative practice is the use of incentive programs. This kind of encouragement is to reward the team instead of the individual. These incentives are based on outcomes and behaviors and could be monetary or nonmonetary (Bankston & Glazer, 2013).
A valid tool has been developed to measure team member perception of collaboration from a variety of diverse health care settings. The tool is called the collaborative practice assessment tool (CPAT). The CPAT is a 57 item tool, based on eight domains with a seven point Likert scale ranging from ‘strongly disagree’ to ‘strongly agree’. Three open ended questions were also included asking for team member’s perception of what worked well and what was felt to be challenges. The intent of the tool is for team members to identify areas of strengths and weakness creating an understanding of how their collaboration functions with the common goal of enhanced patient outcomes (Schroder et al., 2011).
