The patient centered care concept support the active involvement of the patients and their families, in the decision making process that are related upon the treatment services. The IOM (Institute of Medicine) defines patient-centered care as: "Providing care that is respectful of and responsive to individual patient preferences, needs, and values, and ensuring that patient values guide all clinical decisions."

Changes that involve the patients as putting them at the Heart of the system include the access to the electronic health records, and having advance access to their medical care. Implementing an electronic health system is patient centered care if it involves and strengthens the patient and doctor relationship, whilst promoting the communication skills, to help the patient to involve themselves about their health, whilst involving them in their own care. The advances being done within the social change in the medicinal history, the main definition of the patient centered care is evolving since the physician should invite the patient to participate and communicate their concerns, and feedback. In Malta investments are being developed on to a large scale, by developing adequate measures to achieve the intended interventions to improve the patient centered care to achieve their goals. In the patient centered care approach, the patient preferences and thoughts should be respected, and justified, independently to their relationship to the health outcomes.
The patient centered care is prominently on the political agenda. New measures should undergo a cognitive testing and piloting in a variety of settings, recognizing the fact that there is no single measurement that will adequately capture the relevant aspects of the patient centered care, across the Maltese population. To provide an actionable feedback to achieve a continuously patient centered care, surveys, and standardized patient assessments, or direct observations will be vital. A patient centered care must have the following characteristics:-
